Preparing for my first visit:
Welcome! As your dental provider, our primary goal is that you receive the optimal treatment needed to restore or maintain your dental health.
What to expect on your first visit:
Our dental team will welcome you and get your appointment started. You will have a comprehensive exam and any x-rays needed to complete the exam. Dr. Weaver will explain your diagnosis and if any treatment is needed, what your options are.
We will then schedule you in for a prescribed cleaning at your convenience. If there is any treatment needed you will schedule to meet with our financial coordinator to complete a treatment plan.
What do I need to bring to my first visit?
To save time in the waiting room, please print and fill out the New Patient Forms and bring them with you to your appointment. If you are unable to print them, please stop by our office and pick up the forms prior to your appointment.
A current list of any medications you are taking and the name of your health care provider.
Have any current x-rays sent or e-mailed to us from your former dentist prior to your first visit. If we do not receive them, we will have to take new ones to complete the exam and they may not be covered by your insurance.
If you have dental insurance, bring the necessary completed form and your insurance card if you have one. The information on the form is necessary for us to assist you in billing your insurance. Please fill it out completely.
Please alert our office if you have a medical condition that may be of concern prior to your visit. (i.e. recent surgeries, heart conditions, artificial heart valve, artificial joint, diabetes, high blood pressure, etc. Also if your doctor has you pre-medicate prior to dental treatment.)

Starting check-ups early in your child's life can help promote healthy gums and teeth, and good dental hygiene. It's important to prepare them about what to expect at their dental appointments.
Regular dental care helps prevent inconvenient dental emergencies. However, dental emergencies can and do occur. Click here to read about some of the more common dental emergencies and what you can do until you can get to our office. A good rule of thumb: if it hurts, do NOT wait to make an appointment. We will be happy to see you as soon as possible.
